Accordingly, I understand that despite the practitioners’ efforts, my safety may not be able to be ensure at all times from such risks and dangers. I understand that by purchasing this coaching, training, treamtment, therapy, product and/or informational services I herby remove liability from Advanced Therapy and Performance, it’s employees and shareholders. By submitting payment, I am acknowledging that my Medical Doctor has cleared me to exercise without resstriction. I hereby give permission for testimonies, communications, and images of, captured during their time at through video, photo and digital camera, to be used for the purposes of documented progress and promotional material and publications, and waive any rights of compensation or ownership thereto.
